1/10/2023 EnchantaVaughn Approved Behavior: During the visit, client displayed appropriate affect and easy-going behaviors as he engaged in an appropriate manner with his mom, sister, and brother in the home. Client displayed his ability to use positive communication as he expressed things that happened in the day that were meaningful and pleasurable for him. Client showed control over all distractions, which helped him stay focused on the important parts of service. Client displays the correct manners, saying please and excuse me when he had to ask a question. Behaviors were maintained and managed at this time. Client was observed following rules and being compliant of the reinforcement provided by mom and his counselor. Behavior: Client verbalizes positive self-talk and affirmations around his family. Client expresses gratitude and compassion toward his counselor for helping him and being there for him. Client was polite and courteous to mental health professional while reviewing the psycho social activity. Client was applauded for listening, paying attention, and sitting in his seat correctly. Client was receptive to QMHP prompts and directives as he learned about new ways to cope and adapt within his home environment. Client communicated with his counselor that he was having a good day. Mom was happy client was being respectful and expressed joy for his positive behaviors. Client acknowledges his objectives and made a commitment to achieve them daily. Intervention: QMHP provided individual and family counseling based on specific treatment objectives. QMHP initiated a rapport building conversation with client in efforts to learn more of his strengths. QMHP monitored progress in demonstrating individualized pro-social skills (e.g. anger management, education, problem-solving, conflict resolution, appropriate verbalization of feelings, and cognitive-behavioral programming). QMHP recognized client for his hard work, efforts, participation, and working towards improvement. QMHP complimented client for utilizing acquired skills, paying attention, and focusing on the importance of in home services. Intervention: QMHP continued to establish a positive supportive relationship with client. QMHP offered simple praises to the client for behaving appropriately in the home and encouraged client to practice the positive behaviors daily. QMHP supported client displaying positive social interaction with siblings and parent. QMHP listened to client verbalize his feelings towards making good decisions at home and avoiding conflicts. Positive feedback was provided to the client as he displayed a clear understanding to how to use effective communication and assertiveness skills. QMHP encouraged client to maintain his behavior and commended client on his efforts to maintain his ISP objectives throughout the day. Response: Client transitions smoothly and safely to session area, displaying appropriate behaviors. Client smiled and hugged QMHP. Client verbalizes positive feelings about himself and his family. Client interacted well within his home environment by implementing positive conduct and verbalizations. Client discussed mental illness and its causes as a family. Client displayed interest in learning about ADHD, as evidenced by him asking questions and maintaining a cheerful mood. Client reported a decreased stress level due to his modulation of social contact and being able to remain calm. Response: Client has become more involved in age appropriate social activities and was given positive reinforcement for these choices. Client did not display any outbursts during the role play exercise. Client learned to look for similarities rather than differences in others. Client was able to speak calmly and demonstrate his improved communication skills while engaging in self-reflection. Client demonstrated expectations well in the home. Client is making progress with expressing his feelings and showing others respect and courtesy by talking with appropriate tone and initiating healthy conversation.
